Purpose:

We reported a case with intradural lumbar disc herniation vvhich is a rare pathology.

Introduction:

Diagnosis is difficult in preoperative period and easily dismissed during the operation.

Case:

A fifty-five years old man who was admitted to our clinic with lovver extremity paraparesis and urinary retention was presented. L4-5 disc herniation was detected on his radiological investigation and he was operated emergently. İntradural free fragment was found intraoperatively and his neurological deficits relieved totally.

Conclusion:

İntradural disc herniation should be kept in mind in operation of a lumbar disc vvhich is not correlated with clinical and radiological findings, and continuing dural and nerve root retraction difficulty after discectomy.
